This is where you provide a way for people to send you a message, ask questions or indicate preferences for something. The form below is a Google document and extremely simple to set up. For those wanting a little more control then Zoho Creator provide an excellent and comprehensive form creation facility too. The results or data collected gets stored in an on-line database - for the Google version this is a spreadsheet and anyone familiar with Excel will be able to work with the data easily. This design comes in the following low-sodium flavors: red, green, blue, magenta, and brown. Click on a color to see it in action (requires JavaScript).
